How to complete the Supplement B form

Supplement B (formerly known as Section 3) is an I-9 form that must be completed when one of the following occurs [1] :
- Your employee legally changes their name
- Your employee is rehired within 3 years of the date that their original I-9 form was completed
- Your employee’s documentation of employment authorization has expired
If your employee is rehired after 3 years of the date that their original I-9 form was completed, they will be required to complete a new I-9 form. Learn more in this article.
Supplement B Steps
To complete a Supplement B form, follow the steps below:
- Click I-9 Center in the left sidebar.
- Find the employee you need to complete Supplement B for and click Manage to the right of their name.
- Click the More Actions button and then Supplement B.
- Click either the Rehire staff or Change name tab based on the reason you are completing this Supplement B form.
- Complete all required fields including your electronic signature.
- Click Submit.
Important Note:
Supplement B forms do not need to be sent to E-Verify (for companies using E-Verify). Therefore, there are no further actions after the form is submitted [2]. The Fountain I-9 Center will save this form with the worker’s original I-9 form. Additionally, the I-9 Center audit trail for this worker will show a timestamped event for the Supplement B form being filed, including the name of the administrator who filled out and submitted the form. Access the audit trail by clicking More Actions and then Take Action on I-9.
